Passport Appointment Requirements

  1. All applicants must appear in person, including both parents of children under age 16 and one parent for applicants who are aged 16-17.
  2. Arrival on time for the appointment is mandatory.
  3. The application must be typed and/or printed using BLACK ink prior to the appointment.
  4. Proof of citizenship is required via an original/certified birth certificate, original certificate of naturalization, or previous passport.
  5. Proof of parental relationship (original/certified birth certificate) is required for all applicants 15 years of age and under. If the applicant already has a previous passport that will be submitted with the application, a photocopy of the birth certificate is still required.
  6. Original documents will accompany the application.
  7. Two payments will be made. One for the U.S. Department of State; one for the Ventura County Law Library.
    • Payments to the U.S. Department of State must be by check or money order only.
    • The payment to the Ventura County Law Library must be cash only.
