If your valid U.S. passport has been lost/stolen, you must report it to the U.S. Department of State immediately.
Report it:
- Online
- By mail, or
- In person at a passport application acceptance facility when you apply for your new passport.
To receive a new passport, you will schedule an appointment at a passport acceptance facility (like the Law Library) and complete forms DS-11 (the passport application) and form DS-64 (the statement regarding a valid lost or stolen U.S. passport).
If you have completed the DS-64 form online or by mail prior to your in-person appointment, please bring with you a proof of submission or a copy of the DS-64 form.
If your lost passport is expired and therefore no longer valid, you do not need to report it lost/stolen.
