The Disaster Recovery Reform Act of 2018 allows applicants to replace lost passports for free for eligible
disasters.
If your disaster qualifies, you will submit the following through mail:
- DS-5504 (Replacement Passport Form)
- New Passport Photo
- DS-64 (Lost/Stolen Passport Form) with the following:
- Name of the disaster
- Address of the residence where the loss occurred
- Approximate date when the loss occurred
- A statement that the replacement passport fees will not be reimbursed by other sources, such as a homeowner’s insurance policy.
If your disaster does not qualify according to the U.S. Department of State, you will need to schedule an appointment at a passport acceptance facility (such as the Law Library), and complete forms DS-11 and DS-64, paying all required fees.
