Applicants who have never had a U.S. passport issued before will use the DS-11 form, handwritten in black ink or printed.
The following must be taken to your appointment:
- The applicant (all applicants must appear in person, regardless of age)
- Form DS-11, completed in black ink and not yet signed
- Evidence of U.S. citizenship. (Original + Photocopy)
- Photo ID. (Original + Photocopy, front & back, single-sided)
- Passport photo. (We do not offer photo services on-site)
- Two Separate Payments
- (1) For fees paid to the U.S. Department of State, you can submit a CHECK (personal, certified, cashier’s, traveler’s) or MONEY ORDER payable to “U.S. Department of State.”
- (2) The federally mandated $35 (per application) execution fee is a separate payment that must be made out to the Ventura County Law Library in the form of CASH, MONEY ORDER, or CASHIER’S CHECK. (Personal checks are not accepted for the execution fee.)
*Please note that if you have a special circumstance (i.e., a name change since your last passport), additional documentation that is not listed is required. If you have any questions, it is best to check the State Department website.
